Changes in 2014 Relating to PA Birth Certificates and Passports

If you have a short version (just your birth name) of the Commonwealth of Pennsylvania Birth Certificate, you will not be able to apply for a USA Passport. USCIS requires the longer version. The State of Pennsylvania Vital Records is now issuing the longer version, which includes parents' name, DOB's, etc...

If you are going to use the short version birth certificate, USCIS will deny your passport request. To avoid any delays, I strongly recommend that you request for a new PA Birth Certificate.
