For decades, Philadelphia FOP Lodge #5 has recognized our heroic members on the job for their brave and selfless acts protecting and serving our communities. We honor our award recipients with an Awards Ceremony that we host a few times a year in our Heroes Ballroom. We invite our honorees and their families to celebrate with the FOP Executive Board, Committee members as well as Philadelphia Police Department Command Staff.

Our FOP Lodge #5 Awards Committee chooses our honorees from submissions received from our members across all districts and units. Multiple honorees can be recognized for the same job as a part of their team in their district or unit.

We encourage all supervisors to submit any member of any rank from any district or unit for award consideration.

Starting May 1^st, we will no longer be accepting paper submissions and will only accept digital submissions from this form. 

Please note this is for FOP Lodge #5 Awards only and not for the official Philadelphia Police Department/City of Philadelphia Commendation submission. Any Active Police Officer of any rank can submit this form for awards consideration for any job in any district or unit.
