It is very important that you notify FOP Lodge 5 when you have a change of address or name change due to marriage, divorce, etc. We will notify both the State and National FOP Lodges on your behalf. You must always keep your information current for your beneficiaries and insurance policies and always remember to notify LEHB of any personal information changes as well.
